AUSTCHAM BOOKING FORM

Fill in all details including payment on this form and return by fax to 2877 0860 to make a booking or return by email to bookings@austcham.com.hk

Then send this form with payment (cheque made out to 'AustCham' or credit card) to 4/F, Lucky Building, 39 Wellington Street, Central, Hong Kong.

Do not post payment less than three days prior to event - payment is available at the door, for most events.

Payment MUST be made prior to entry of event - entry may be refused if payment not received.

A completed form is a confirmed booking UNLESS otherwise contacted. No shows will be charged.

Receipts will only be issued at the event. For more details call 2522-5054.

Cancellations must be faxed to AustCham 48 hours prior to the event and will be confirmed by telephone.
